Effective September 1st, 2025 Strength In You OBGYN will begin charging a fee for completion of certain medical forms such as Family Medical Leave Act (FMLA), Disability Applications, and Letters of Support for Immigration. Other charges for forms will be at the discretion of the office.
Fee Schedule
$25 due upon submission of the forms. Forms will be completed 7 business days after being received
An additional $20 will be due if there is a request for the forms to be expedited. Forms will be completed by the 2nd business day the form is received.
Form completion will not be initiated until the fee is paid in full.
